Community Health Representatives are trusted frontline public health workers. through facilitating access to quality and culturally competent services, they are a link between health and social services and the communities they serve.

Services Provided

Medication Delivery

If requested, CHR will pick up patients’ prescriptions from the PIHC pharmacy and deliver them to their residence. CHR does not pick up or deliver controlled substances. Medication requested after 2:00 PM will be delivered the following day. The person receiving the medication must be an adult and must sign for the medication.

Welfare Checks & Home Visits

Welfare checks are conducted bi-weekly or monthly for our high-risk patients, via telephone. Home visits are conducted as needed where a CHR can provide environmental /home safety assessments, light housework, assist with ADLs, emotional support, referrals, and/or monitor a patient’s health status.

Car Seat Courses

To qualify to receive a car seat one must first get a referral from PIHC. Once CHR has received the referral we will schedule a one-on-one car seat class here at our building. When the class is completed, the parent will receive a car seat.

Optical Services

The CHR Optical Office, located at PIHC, is open and sees clients by appointment only.

No walk-ins will be accepted.

The Optical office is closed on Tuesdays and Thursdays.

Durable Medical Equipment (DME)

To qualify for this service, one must be seen by a physician at PIHC and get a referral for the DME that’s required. If insurance doesn’t cover the expenses, and if the equipment is available, CHR will approve and deliver the DME to the patient’s residence.

Transportation Services

CHR offers transportation to and from FMC located in Parker, as well as FMC located in Lake Havasu City. Call CHR for more information.

CHR offers non-emergency medical appointment transportation both locally and out of town.

Local Transportation

CHR does not transport to the Emergency Room

Please call (at least) 2 hours in advance to schedule local transportation.

Out-of-town Transportation

Please call (at least) 5 business days in advance to schedule out-of-town transportation.

When scheduling medical appointment transportation be sure to notify CHR if a wheelchair van, regular van, car, or an escort is required for the transport. One escort may accompany the patient during the transport and appointment.

Patients must provide their own car seats for transport if required.

An escort is a person over the age of 18 who is able and willing to assist the patient during their appointment. Please call ahead if you need to cancel a scheduled transport.
